Assessing the effectiveness of human papillomavirus (HPV) vaccination to prevent cervical cancer: perspectives from Germany

For approximately 2 years now, cervical cancer has been “converted” from an oncological disease to an infectious disease, which is said to be preventable by and large by two vaccines licensed in many countries. However, human papillomavirus (HPV) vaccines differ from existing others, as the former target
a condition which only in a minute fraction of infections will lead to serious consequences, but after a long(er) latency period. Furthermore, it should be kept in mind that in clinical trials, the quadrivalent vaccine was tested in fewer than 1200 girls 16 years and younger.1

Experts call for more cervical cancer tests

SEXUALLY active females, even those classified as “children”, should get screened as a precaution against cervical cancer.

According to Professor Kimlin Tam Ashing, Professor, Department of Population Sciences and Director of the Centre of Community Alliance for Research and Education, City of Hope US, screening should start among females 18 years-old. However, she qualified this statement saying it should take place at the time of sexual initiation.

“Unfortunately, that could be 12 or 14 so at that age if the child, she is a child, has begun sexual activity she really needs to be getting a pap test,” she said at the launch of the research study, “Role of Stigma in Cervical Cancer Screening in Trinidad and Tobago” at the Carlton Savannah. Among its aims is to identify the barriers to cervical cancer screening.

Cancer is the second leading cause of death in TT and data from the National Cancer Registry indicated that cervical cancer is the second leading cancer among women. Breast cancer ranks first. The pap smear test can be done annually or more frequently depending on what is found in the cells. Cervical cancer is caused by the human papilloma virus (HPV) which is sexually transmitted and can cause cancerous cellular changes which affect the area at the opening of the uterus.

HPV4 not associated with autoimmune conditions

BOSTON — The quadrivalent human papillomavirus vaccine did not raise the risk for developing autoimmune conditions, according to data presented here at the 50th Interscience Conference on Antimicrobial Agents and Chemotherapy.

In a study sponsored by Merck and conducted on behalf of the Gardasil Safety Team, researchers investigated the safety of the quadrivalent HPV vaccine (HPV4; Gardasil, Merck). The researchers honed in on identifying 16 prespecified rheumatologic, endocrine and neurologic or ophthalmologic autoimmune conditions.

During the post-licensure, observational study, the researchers monitored girls and women in managed care organizations who received at least one dose of HPV4 between August 2006 and March 2008. Follow-up lasted for 6 months after administration of each dose.

The researchers found 11 conditions among a random sample of girls and women who had at least a 12-month health care plan before HPV4 vaccination. Clinicians reviewed medical records to verify diagnosis and determine time of disease onset. Incidence rates for each condition were also established for vaccinated and unvaccinated participants during case identification and review.

What is Wrong with the Gardasil Girls: Kahlia’s Story

Kahlia spent 2009 as a rotary exchange student in the United States as an active teen and an ambassador for her country. She had dreams and goals just like any adolescent girl her age. Those dreams came to a halt when Kahlia returned to New Zealand and had her first Gardasil injection.

A month later she was in the hospital. Just like other girls adversely affected by Gardasil, Kahlia had to withdraw from school. Her story is below:

On January 27, 2010, I had my first vaccination against cervical cancer. I started to experience headaches and nausea, which I did not relate to the vaccine at first. Over the next three weeks, I experienced uncontrollable mood swings, fatigue, insomnia, and clumsiness. I passed out a week before I was hospitalized.

On the night of February 27, I was hospitalized with tight, stabbing chest pains, shortness of breath, and headaches. My left leg had started spasming and I had a heavy feeling on my left side. After X-rays, ECG and blood tests all came back clear, I was sent home.

NCI awards $13.5 million HPV testing contract to Dutch Diagnostic Lab: What’s wrong with this picture?

The National Cancer Institute (NCI) announced on August 2, 2010 that it has awarded a $13.5 million contract to a Dutch diagnostic laboratory to perform reliable human papillomavirus (HPV) genotyping using a polymerase chain reaction (PCR) system with short target sequences for the cervical biopsy samples collected at the NCI.

Such reliable HPV genotyping technology was not used by the drug manufacturers in validating the efficacy of the genotype-specific HPV vaccines during the clinical trials required for Food and Drug Administration (FDA) approval, and has been rejected by the FDA for commercial distribution to clinical laboratories in the U.S. for general patient care.

This NCI contract confirms that the current FDA-approved HPV test kits for patient care in the U.S. are not reliable.

According to the pre-solicitation notice, HPV infections are very common in sexually active women and only a small subset progress to cervical cancer. When precancerous lesions appear, they may contain multiple types of HPV. It is difficult with the currently available FDA-approved HPV tests to determine whether the HPV strains present are responsible for causing the lesions, or simply there as a passenger.
