If you have a child who has not been vaccinated, or has had limited vaccinations, visit the site below to complete a brief survey regarding the current state of your child’s health. He is attempting to create a database for comparison of vaccinated versus unvaccinated children. The survey is available in several languages. It does not matter what country you reside in.
